Empowering Minds Behavioral Health offers outpatient treatment for Substance Use Disorders (SUD) and Medication-Assisted Treatment (MAT) for opioid dependence, structured through an Intensive Outpatient Program (IOP). Our licensed clinicians use the American Society of Addiction Medicine (ASAM) criteria to establish the level of care appropriate to meet the patient’s needs.

Diagnostic Assessment
A comprehensive evaluation used in behavioral health to understand an individual’s mental, emotional, and behavioral functioning. This integrated approach serves as the foundation for accurate diagnosis and personalized treatment planning.
Walk-In Services for Behavioral Health Treatment
To complete a walk-in assessment, visit our office located at 3170 Central Avenue in Toledo. We provide assessments on a first-come, first-served basis starting at 8:00 a.m. Monday through Friday or call (419) 214-5490 to schedule an appointment.
Medication Assisted Treatment (MAT) Program
The MAT program combines pharmacological medications Buprenorphine (Suboxone or Subutex), an opioid agonist/antagonist that blocks other narcotics while reducing withdrawal risk; with medical oversight, counseling and behavioral therapies to treat opioid and alcohol use disorders.
Intensive Outpatient Program (IOP)
Flexible, multi-tiered treatment programs designed to support recovery while accommodating work, school, and family commitments. Our clinical team engages in multidisciplinary case consultations to evaluate progress and transition individuals to the appropriate group or level of care.
Outpatient Program (OP)
Weekly therapy sessions are structured to support long-term recovery, relapse prevention, and emotional well-being. Our licensed professionals are committed to your sustained progress, utilizing evidence-based practices and personalized treatment plans tailored to your unique needs.
Mental Health Day Treatment
Day treatment is a group-based service that addresses acute mental health challenges, life skills development, and emotional well-being. Group services are available for all ages and are developed to support the needs and enhance life skills for each participant.
Integrated Treatment
Comprehensive, evidence-based services tailored for individuals with co-occurring substance use and mental health disorders, promoting holistic wellness and sustained recovery. Each service is customized to address and support the unique needs of the individual.
Individual Therapy
Individualized sessions with licensed professionals focused on personal recovery goals and emotional well-being. Our experienced team is committed to guiding each client toward a personalized path to health and wellness.
Case Management
Comprehensive care coordination services that connect individuals with essential community resources and ongoing support. Assistance includes securing housing, accessing food programs, enrolling in Medicaid, and facilitating referrals to additional services aligned with each individual’s treatment goals and long-term wellness.
Medical and Nursing Services

Aftercare
Our aftercare services offer ongoing support for individuals transitioning from treatment to sustained recovery. Through weekly group therapy, individual sessions, and case management, we address emotional, personal, and practical needs. Services are flexible, with therapy and medical appointments typically held at our center, while case management and select sessions may be provided in-home for added convenience.
